Narrative:

Improper armed passenger procedures. 5 mins prior to departure, passenger agent entered cockpit with paperwork for armed agent with prisoner. Introduced me to the agent while I was in my seat. I said it was ok to board, forgetting that flight manual requires boarding prisoners first. Agent said there was another agent traveling too, but he was not armed. We were in the midst of predep checklist, and I did not notice the paperwork was missing the phone number to the local agency office. 2 hours into the flight, flight attendant reported that the prisoner was seated in xx (the paperwork stated seat yy), that agent #1 was seated in zz and was asleep and the second agent was in ww and also had his eyes closed. The flight attendant also noted the prisoner was not wearing handcuffs. I told the flight attendant to tell the agent to put the cuffs on the prisoner. The agent said they did not have any adding the prisoner was actually a parolee. He provided papers, but they said the prisoner was to be released to a half-way house. Though neither the prisoner nor agents caused a problem on the flight, I know that in june the FAA issued a security directive (108-00-02A) regarding armed passenger. The air carrier altered its procedures to comply. Given the new directive/procedures, I am concerned about compliance with the directive/procedures. On this flight alone, we had: 1) incomplete paperwork, 2) prisoner changed seats, 3) sleeping armed guards, 4) boarding after passenger, 5) no handcuffs on prisoner, and 6) guards had no restraint devices. This does not provide much assurance that the agents and security department in mia handled this properly or that he had been briefed and was aware of requirements.

Title: MD80. ARMED AGENT AND PRISONER WERE IMPROPERLY BOARDED, HAD INCOMPLETE DOCUMENTATION, AND CONDUCTED THEMSELVES IMPROPERLY ON THE FLT.

Narrative: IMPROPER ARMED PAX PROCS. 5 MINS PRIOR TO DEP, PAX AGENT ENTERED COCKPIT WITH PAPERWORK FOR ARMED AGENT WITH PRISONER. INTRODUCED ME TO THE AGENT WHILE I WAS IN MY SEAT. WE WERE IN THE MIDST OF PREDEP CHKLIST, AND I DID NOT NOTICE THE PAPERWORK WAS MISSING THE PHONE NUMBER TO THE LCL AGENCY OFFICE. 2 HRS INTO THE FLT, FLT ATTENDANT RPTED THAT THE PRISONER WAS SEATED IN XX (THE PAPERWORK STATED SEAT YY), THAT AGENT #1 WAS SEATED IN ZZ AND WAS ASLEEP AND THE SECOND AGENT WAS IN WW AND ALSO HAD HIS EYES CLOSED. THE FLT ATTENDANT ALSO NOTED THE PRISONER WAS NOT WEARING HANDCUFFS. I TOLD THE FLT ATTENDANT TO TELL THE AGENT TO PUT THE CUFFS ON THE PRISONER. THE AGENT SAID THEY DID NOT HAVE ANY ADDING THE PRISONER WAS ACTUALLY A PAROLEE. HE PROVIDED PAPERS, BUT THEY SAID THE PRISONER WAS TO BE RELEASED TO A HALF-WAY HOUSE. THOUGH NEITHER THE PRISONER NOR AGENTS CAUSED A PROB ON THE FLT, I KNOW THAT IN JUNE THE FAA ISSUED A SECURITY DIRECTIVE (108-00-02A) REGARDING ARMED PAX. THE ACR ALTERED ITS PROCS TO COMPLY. GIVEN THE NEW DIRECTIVE/PROCS, I AM CONCERNED ABOUT COMPLIANCE WITH THE DIRECTIVE/PROCS. ON THIS FLT ALONE, WE HAD: 1) INCOMPLETE PAPERWORK, 2) PRISONER CHANGED SEATS, 3) SLEEPING ARMED GUARDS, 4) BOARDING AFTER PAX, 5) NO HANDCUFFS ON PRISONER, AND 6) GUARDS HAD NO RESTRAINT DEVICES. THIS DOES NOT PROVIDE MUCH ASSURANCE THAT THE AGENTS AND SECURITY DEPT IN MIA HANDLED THIS PROPERLY OR THAT HE HAD BEEN BRIEFED AND WAS AWARE OF REQUIREMENTS.
