Narrative:

Upon deplaning at dfw, I noticed a therapeutic oxygen container in the first class closet. My flight attendant advised me that it was boarded at the last moment prior to departure. I was never provided a form to alert me to its presence. This is another case I fear of getting the airplane out regardless of procedures that should be adhered to.

Title: A B757-200 CAPT RPT ON THE LAST MIN BOARDING OF AN OXYGEN CONTAINER WITHOUT THE REQUIRED ACCOMPANYING FORMS FOR HAZMAT AT SJC, CA.

Narrative: UPON DEPLANING AT DFW, I NOTICED A THERAPEUTIC OXYGEN CONTAINER IN THE FIRST CLASS CLOSET. MY FLT ATTENDANT ADVISED ME THAT IT WAS BOARDED AT THE LAST MOMENT PRIOR TO DEP. I WAS NEVER PROVIDED A FORM TO ALERT ME TO ITS PRESENCE. THIS IS ANOTHER CASE I FEAR OF GETTING THE AIRPLANE OUT REGARDLESS OF PROCS THAT SHOULD BE ADHERED TO.
