Narrative:

Air carrier flight xx of jun/mon/00 was operated detroit (dtw) to tokyo (nrt). Scheduled flight time was 12 hours 50 mins block to block. Due to the length of the flight, our company uses 2 cockpit crews, 2 capts and 2 first officer's. Crew change this flight was near nome, alaska. We were dispatched with the #2 main tank aft fuel boost pump inoperative. Air carrier dispatch and both capts agreed on restrs in MEL, primary operational restr was to land with at least 8610 pounds of fuel in each of the 4 main tanks. Passing 170E, air carrier tokyo dispatch assumed operational control of the flight. Somewhat later we were rereleased from short release on to nrt with fuel onboard. Abeam sapporo japan with approximately 1 hour 10 mins flight time remaining, tokyo dispatch advised of additional restr -- the 8610 pounds of fuel in each main tank is to be considered unusable per the MEL. It is now too late to comply with this restr. We continue on to nrt, land in VFR, block in with fuel remaining of 8600/8700/8600/8600 pounds (34500 pounds total). The MEL note of 34440 pounds considered unusable was missed by all 4 cockpit crew, dispatch, nrt dispatch, and planning. The item was caught by the nrt dispatcher planning the next flight of this aircraft.
