Narrative:

On the night of jan/xa/00, I was assigned aircraft xyz as one of my aircraft to work on that night. While making out my crew assignments, a gentleman from vendor was introduced to me. I was told that he would like to try a new radome on an F100 for engineering. I told him and my supervisor XXX WWW that due to the heavy workload I would be unable to help him with manpower. Then during the following conversations with my supervisor XXX WWW said that he would oversee that the paperwork for removing the old radome installing the engineered radome and then reinstalling the original radome back on the aircraft would be taken care of by him (XXX WWW). We have had engineers come and do things on aircraft before. As I said before, we had a heavy workload that night and aircraft xyz had floor board problems and an early trip in the morning. At the end of the shift we are trying to get aircraft back in service, and getting them to the gate on time things get real busy. That morning was no exception, and when I checked over the paperwork and signed the aircraft release for aircraft xyz, I was making sure that all the signature blocks had signatures and employee numbers and did not notice the radome situation that XXX WWW said he would take care of was not on the new routing items for aircraft xyz. I have since found out that it was not and should have been.

Title: A FOKKER 100 WAS DISPATCHED IN NON COMPLIANCE WITH THE RADOME REMOVED AND REPLACED BUT NOT ENTERED INTO THE ACFT LOGBOOK OR JOB CARD SIGNOFFS.

Narrative: ON THE NIGHT OF JAN/XA/00, I WAS ASSIGNED ACFT XYZ AS ONE OF MY ACFT TO WORK ON THAT NIGHT. WHILE MAKING OUT MY CREW ASSIGNMENTS, A GENTLEMAN FROM VENDOR WAS INTRODUCED TO ME. I WAS TOLD THAT HE WOULD LIKE TO TRY A NEW RADOME ON AN F100 FOR ENGINEERING. I TOLD HIM AND MY SUPVR XXX WWW THAT DUE TO THE HVY WORKLOAD I WOULD BE UNABLE TO HELP HIM WITH MANPOWER. THEN DURING THE FOLLOWING CONVERSATIONS WITH MY SUPVR XXX WWW SAID THAT HE WOULD OVERSEE THAT THE PAPERWORK FOR REMOVING THE OLD RADOME INSTALLING THE ENGINEERED RADOME AND THEN REINSTALLING THE ORIGINAL RADOME BACK ON THE ACFT WOULD BE TAKEN CARE OF BY HIM (XXX WWW). WE HAVE HAD ENGINEERS COME AND DO THINGS ON ACFT BEFORE. AS I SAID BEFORE, WE HAD A HVY WORKLOAD THAT NIGHT AND ACFT XYZ HAD FLOOR BOARD PROBS AND AN EARLY TRIP IN THE MORNING. THAT MORNING WAS NO EXCEPTION, AND WHEN I CHKED OVER THE PAPERWORK AND SIGNED THE ACFT RELEASE FOR ACFT XYZ, I WAS MAKING SURE THAT ALL THE SIGNATURE BLOCKS HAD SIGNATURES AND EMPLOYEE NUMBERS AND DID NOT NOTICE THE RADOME SIT THAT XXX WWW SAID HE WOULD TAKE CARE OF WAS NOT ON THE NEW ROUTING ITEMS FOR ACFT XYZ. I HAVE SINCE FOUND OUT THAT IT WAS NOT AND SHOULD HAVE BEEN.
