Narrative:

In good conscience, I felt there were no leaks when engine #3 was leak checked in ZZZ. Ship xyz arrived fuel dripping from #3 engine cowling. Opened cowling, ran engine at idle -- no leaks noted. Closed cowling, signed off logbook. On follow-up on jan/xa/00, maintenance in XXX found #3 fuel flow regulator o-ring crushed.
