Narrative:

On the morning of nov/xa/99 I was flying the ILS runway 19 approach into teb. The WX was very poor with a 300 ft overcast ceiling and an RVR of somewhere around 3500 ft. The approach has a decision ht of 300 ft AGL, so there was no extra room to spare. The problem arose when the controller asked me to increase my speed for a jet that was following me. The next problem was that I received a poor vector onto the final approach course. I had full scale deflection on the localizer until just prior to the OM. When the needle finally stated to move, I started my descent but was too fast and could not intercept the GS. At that point I decided to go missed approach. The ILS runway 19 is a new approach with a very complicated missed approach procedure. It requires an immediate climbing right turn to 1500 ft via heading 205 degrees and teb VOR/DME 278 degree radial to trova/4.7 DME/radar vectors, then climb to 3000 via teb 278 degree radial to morns intersection and hold. The problem was when I went missed, I was not at the normal missed approach point, so instead of turning I tried to reintercept the localizer to get to the point where the turn should start. Another factor was that my aircraft can only hold 1 frequency at a time for the VOR, so by the time I switched over to the VOR radial on teb, I had passed it. I immediately turned to intercept. Some contributing factors were that the new york controller was unwilling to give me a heading to fly after he noticed that I had passed the radial nor did he inform me that I was off course, he just kept asking me what my heading was. He finally gave me a heading to fly and vectored me around for the same approach and it went fine. The very next aircraft behind me also went missed and had even more problems on the procedure coming even closer to another aircraft. This procedure (ILS runway 19 missed approach teb) should be reviewed and maybe changed to be more user friendly. The ATC controller should also be more helpful in preventing a close call. We should also review the missed approach procedures for other approachs in that area.

Title: PLT OF AN ATX CARGO PIPER PA60 AEROSTAR FAILED TO FOLLOW THE PUBLISHED MISSED APCH PROC RESULTING IN LOSS OF SEPARATION WITH A B737-500 FOLLOWING ON THE APCH.

Narrative: ON THE MORNING OF NOV/XA/99 I WAS FLYING THE ILS RWY 19 APCH INTO TEB. THE WX WAS VERY POOR WITH A 300 FT OVCST CEILING AND AN RVR OF SOMEWHERE AROUND 3500 FT. THE APCH HAS A DECISION HT OF 300 FT AGL, SO THERE WAS NO EXTRA ROOM TO SPARE. THE PROB AROSE WHEN THE CTLR ASKED ME TO INCREASE MY SPD FOR A JET THAT WAS FOLLOWING ME. THE NEXT PROB WAS THAT I RECEIVED A POOR VECTOR ONTO THE FINAL APCH COURSE. I HAD FULL SCALE DEFLECTION ON THE LOC UNTIL JUST PRIOR TO THE OM. WHEN THE NEEDLE FINALLY STATED TO MOVE, I STARTED MY DSCNT BUT WAS TOO FAST AND COULD NOT INTERCEPT THE GS. AT THAT POINT I DECIDED TO GO MISSED APCH. THE ILS RWY 19 IS A NEW APCH WITH A VERY COMPLICATED MISSED APCH PROC. IT REQUIRES AN IMMEDIATE CLBING R TURN TO 1500 FT VIA HDG 205 DEGS AND TEB VOR/DME 278 DEG RADIAL TO TROVA/4.7 DME/RADAR VECTORS, THEN CLB TO 3000 VIA TEB 278 DEG RADIAL TO MORNS INTXN AND HOLD. THE PROB WAS WHEN I WENT MISSED, I WAS NOT AT THE NORMAL MISSED APCH POINT, SO INSTEAD OF TURNING I TRIED TO REINTERCEPT THE LOC TO GET TO THE POINT WHERE THE TURN SHOULD START. ANOTHER FACTOR WAS THAT MY ACFT CAN ONLY HOLD 1 FREQ AT A TIME FOR THE VOR, SO BY THE TIME I SWITCHED OVER TO THE VOR RADIAL ON TEB, I HAD PASSED IT. I IMMEDIATELY TURNED TO INTERCEPT. SOME CONTRIBUTING FACTORS WERE THAT THE NEW YORK CTLR WAS UNWILLING TO GIVE ME A HDG TO FLY AFTER HE NOTICED THAT I HAD PASSED THE RADIAL NOR DID HE INFORM ME THAT I WAS OFF COURSE, HE JUST KEPT ASKING ME WHAT MY HDG WAS. HE FINALLY GAVE ME A HDG TO FLY AND VECTORED ME AROUND FOR THE SAME APCH AND IT WENT FINE. THE VERY NEXT ACFT BEHIND ME ALSO WENT MISSED AND HAD EVEN MORE PROBS ON THE PROC COMING EVEN CLOSER TO ANOTHER ACFT. THIS PROC (ILS RWY 19 MISSED APCH TEB) SHOULD BE REVIEWED AND MAYBE CHANGED TO BE MORE USER FRIENDLY. THE ATC CTLR SHOULD ALSO BE MORE HELPFUL IN PREVENTING A CLOSE CALL. WE SHOULD ALSO REVIEW THE MISSED APCH PROCS FOR OTHER APCHS IN THAT AREA.
