Narrative:

Aircraft landed in ZZZ, us, about XA35. At this time the captain wrote up the first officer's push-to-talk switch. Maintenance control was informed that the aircraft required a jack for a microphone for the first officer. Maintenance control told the maintenance worker to call back if the aircraft was not equipped with a jack. This person never called maintenance back and it was assumed that the aircraft was properly equipped by maintenance control and myself. It was found that there was no jack equipped in the aircraft at around XG00. The aircraft was promptly grounded until the aircraft was properly fixed. Between the time the MEL was written and the time the mistake was found, the aircraft was dispatched and flown for 2 revenue flts. From the point of view of myself and maintenance control, we should have followed up on the problem. If the worker did not call back, we should have called him to make sure the situation was correctly taken care of.

Title: A LIGHT TRANSPORT, LOW WING, TURBOPROP WAS DISPATCHED IN NON COMPLIANCE FOR 2 REVENUE FLTS WITH THE FO'S PUSH-TO-TALK SWITCH DEFERRED IN CONFLICT WITH THE MEL.

Narrative: ACFT LANDED IN ZZZ, US, ABOUT XA35. AT THIS TIME THE CAPT WROTE UP THE FO'S PUSH-TO-TALK SWITCH. MAINT CTL WAS INFORMED THAT THE ACFT REQUIRED A JACK FOR A MIKE FOR THE FO. MAINT CTL TOLD THE MAINT WORKER TO CALL BACK IF THE ACFT WAS NOT EQUIPPED WITH A JACK. THIS PERSON NEVER CALLED MAINT BACK AND IT WAS ASSUMED THAT THE ACFT WAS PROPERLY EQUIPPED BY MAINT CTL AND MYSELF. IT WAS FOUND THAT THERE WAS NO JACK EQUIPPED IN THE ACFT AT AROUND XG00. THE ACFT WAS PROMPTLY GNDED UNTIL THE ACFT WAS PROPERLY FIXED. BTWN THE TIME THE MEL WAS WRITTEN AND THE TIME THE MISTAKE WAS FOUND, THE ACFT WAS DISPATCHED AND FLOWN FOR 2 REVENUE FLTS. FROM THE POINT OF VIEW OF MYSELF AND MAINT CTL, WE SHOULD HAVE FOLLOWED UP ON THE PROB. IF THE WORKER DID NOT CALL BACK, WE SHOULD HAVE CALLED HIM TO MAKE SURE THE SIT WAS CORRECTLY TAKEN CARE OF.
