Narrative:

On nov/xe/99 I received a letter from the FAA notifying me of an incident that occurred on sep/xa/99 involving an inoperative 'no smoking/fasten seat belt' sign aboard an air carrier aircraft, serial number xyz. I operated this aircraft on sep/xe/99 on air carrier flight abc from ZZZ to madison. At no time during the course of events that led up to the operation of this flight was I, as PIC, notified of any discrepancies by any crew member. Preflight preparation, including the cabin of the aircraft, was performed by a qualified crew member with no discrepancies or inoperative components brought to my attention. Flight abc was operated normally. The flight was routine and uneventful.

AT NO TIME DURING THE COURSE OF EVENTS THAT LED UP TO THE OP OF THIS FLT WAS I, AS PIC, NOTIFIED OF ANY DISCREPANCIES BY ANY CREW MEMBER. PREFLT PREPARATION, INCLUDING THE CABIN OF THE ACFT, WAS PERFORMED BY A QUALIFIED CREW MEMBER WITH NO DISCREPANCIES OR INOP COMPONENTS BROUGHT TO MY ATTN. FLT ABC WAS OPERATED NORMALLY. THE FLT WAS ROUTINE AND UNEVENTFUL.
