Narrative:

(Before starting ASR, radio did not receive 2 calls but seemed ok after.) CFI and student inbound on ASR approach. First ASR for student. Controller gives MDA and missed approach instructions with missed approach point. Controller is one-on-one with aircraft, so no readback. Also gives the pilot the after completion instructions. CFI tells student to write them down because else he would forget them. Student writes down the wrong thing. 3 SM from runway, CFI tells controller airport in sight and before this was reported the controller told student and CFI to go missed at missed approach point. Gets the clearance to land and/or touch-and-go. CFI and student keep on descending to MDA for practice and CFI gets distraction by student which is asking him if he has to go down and land and take some flaps. While ATC is talking, CFI is not sure what to do and of course you should ask ATC for clarification, but somehow you don't do it. Student and CFI go down and land and see another plane ahead of them close to the ground (probably taking off). After takeoff, we (student and CFI) turn up to our ('assigned') heading and we were told to call up the tower after we would touch down in conway. I (CFI) was told that I did not go missed approach when I was told to and I told the myrtle beach supervisor that I was not sure about what we were supposed to do and my regrets. One thing is for sure -- always ask ATC if you're not sure what to do! Don't let anything come in between.

AFTER TKOF, WE (STUDENT AND CFI) TURN UP TO OUR ('ASSIGNED') HDG AND WE WERE TOLD TO CALL UP THE TWR AFTER WE WOULD TOUCH DOWN IN CONWAY. I (CFI) WAS TOLD THAT I DID NOT GO MISSED APCH WHEN I WAS TOLD TO AND I TOLD THE MYRTLE BEACH SUPVR THAT I WAS NOT SURE ABOUT WHAT WE WERE SUPPOSED TO DO AND MY REGRETS.
