Narrative:

I failed to receive ATC clearance prior to takeoff out of clt for a flight to ilm. I always make sure while at the gate that I zero out the 4 transponder digits so as to avoid doing this. I had been flying this flight repeatedly during the month, so I knew the standard route and SID altitudes. This, in conjunction with complacency and a very quick and hectic taxi out, allowed me to miss the zeroed out transponder code and the fact that I had failed to receive clearance while at the gate. On being switched to clt departure, the controller reminded me to squawk the proper transponder code (about 30 seconds after takeoff). This was when I realized my mistake. I then used the pre departure clearance to pick up the clearance and the flight then proceeded normally. ATC may not have even noticed. Clearance was received no more than 60 seconds after takeoff. I can avoid this in the future by not allowing myself to be rushed to the point of missing anything, and by applying myself completely to my duties even when on a flight that I am very familiar with.

Title: FK10 CREW DEPARTED ARPT WITHOUT RECEIVING THEIR IFR CLRNC.

Narrative: I FAILED TO RECEIVE ATC CLRNC PRIOR TO TKOF OUT OF CLT FOR A FLT TO ILM. I ALWAYS MAKE SURE WHILE AT THE GATE THAT I ZERO OUT THE 4 XPONDER DIGITS SO AS TO AVOID DOING THIS. I HAD BEEN FLYING THIS FLT REPEATEDLY DURING THE MONTH, SO I KNEW THE STANDARD RTE AND SID ALTS. THIS, IN CONJUNCTION WITH COMPLACENCY AND A VERY QUICK AND HECTIC TAXI OUT, ALLOWED ME TO MISS THE ZEROED OUT XPONDER CODE AND THE FACT THAT I HAD FAILED TO RECEIVE CLRNC WHILE AT THE GATE. ON BEING SWITCHED TO CLT DEP, THE CTLR REMINDED ME TO SQUAWK THE PROPER XPONDER CODE (ABOUT 30 SECONDS AFTER TKOF). THIS WAS WHEN I REALIZED MY MISTAKE. I THEN USED THE PDC TO PICK UP THE CLRNC AND THE FLT THEN PROCEEDED NORMALLY. ATC MAY NOT HAVE EVEN NOTICED. CLRNC WAS RECEIVED NO MORE THAN 60 SECONDS AFTER TKOF. I CAN AVOID THIS IN THE FUTURE BY NOT ALLOWING MYSELF TO BE RUSHED TO THE POINT OF MISSING ANYTHING, AND BY APPLYING MYSELF COMPLETELY TO MY DUTIES EVEN WHEN ON A FLT THAT I AM VERY FAMILIAR WITH.
