Narrative:

At end of tango arrival zrh, proceeded beyond clearance limit without any clearance for a variety of reasons, fatigue, rushed, behind the descent, hand-flying, cockpit distractions, scenery, relatively inexperienced at the airport, lack of good CRM between first officer and captain, in that both pilots thought about not proceeding but did not communicate their concerns to each other. Captain closed a route discontinuity that would have prevented it. The STAR is clearly depicted, but in the rushed descent and hand flown environment, I missed the depiction. In the future I will brief stars and approachs at TOD, not just the approachs.
