Narrative:

I was first officer on flight on mar/xa/99 from ord to sfo. Our B747-200 blocked out at ord. The flight was uneventful until approach to sfo and was observed by an FAA inspector, the poi. I was the PNF and was handling radio communications. The WX was cavok with winds around 280 degrees 15 KTS. We were held at 7000 ft and until 18 DME and then finally cleared for the quiet bridge approach to runway 28R. The captain configured and slowed to get down to profile. Bay approach asked us to sidestep to runway 28L and we complied to give us a little more time. Bay was talking to a cessna with a 'lifeguard' call sign going to the right runway. While maneuvering to runway 28L a B757 (I think) came at us from the left. He was on a heading from bay. We got an RA to climb and he responded to descend I think. We missed by 500 ft I guess. On final, finally, we got another TA when the lifeguard cessna got too close. My guess is bay got focused on the lifeguard and forgot about the B757 setting up an ugly situation. Sfo is a real problem!

MY GUESS IS BAY GOT FOCUSED ON THE LIFEGUARD AND FORGOT ABOUT THE B757 SETTING UP AN UGLY SIT. SFO IS A REAL PROB!
