Narrative:

On this night in ZZZ, deicing was proceeding and I was called on a list of qualified people from the heavy maintenance list. I had just finished my deicing training required several days before. I was assigned the west deicing pad on the airport. When we got there we found that neither truck's radio was working. We proceeded to deice the first aircraft in line using visual communication. He had been waiting a while so the pilot was naturally agitated. When we were done deicing, I forgot that he had to have a deicing report read back to him and so we established voice communication using a headset. As I began talking to the pilot it was obvious he was upset and so I asked him what it was he needed. He told me, and I told him that I was not exactly sure what he needed so I went and talked to the other deice truck driver and he gave me the information we needed. He got the impression through our talk that I was not trained properly. It was merely a communication breakdown and nothing more. If he got that impression from me, I am truly sorry.

Title: CAPT OF AN MD80 HAD TO REMIND A&P MECH DEICER THAT HE NEEDED A DEICE RPT AFTER HIS ACFT WAS DEICED. THE DEICER HAD JUST RECEIVED DEICE TRAINING AND DID NOT HAVE EXPERIENCE WITH DEICE RPT PROCS OR THE USUAL AVAILABLE RADIO MEANS WITH WHICH TO COM WITH THE FLC.

Narrative: ON THIS NIGHT IN ZZZ, DEICING WAS PROCEEDING AND I WAS CALLED ON A LIST OF QUALIFIED PEOPLE FROM THE HVY MAINT LIST. I HAD JUST FINISHED MY DEICING TRAINING REQUIRED SEVERAL DAYS BEFORE. I WAS ASSIGNED THE W DEICING PAD ON THE ARPT. WHEN WE GOT THERE WE FOUND THAT NEITHER TRUCK'S RADIO WAS WORKING. WE PROCEEDED TO DEICE THE FIRST ACFT IN LINE USING VISUAL COM. HE HAD BEEN WAITING A WHILE SO THE PLT WAS NATURALLY AGITATED. WHEN WE WERE DONE DEICING, I FORGOT THAT HE HAD TO HAVE A DEICING RPT READ BACK TO HIM AND SO WE ESTABLISHED VOICE COM USING A HEADSET. AS I BEGAN TALKING TO THE PLT IT WAS OBVIOUS HE WAS UPSET AND SO I ASKED HIM WHAT IT WAS HE NEEDED. HE TOLD ME, AND I TOLD HIM THAT I WAS NOT EXACTLY SURE WHAT HE NEEDED SO I WENT AND TALKED TO THE OTHER DEICE TRUCK DRIVER AND HE GAVE ME THE INFO WE NEEDED. HE GOT THE IMPRESSION THROUGH OUR TALK THAT I WAS NOT TRAINED PROPERLY. IT WAS MERELY A COM BREAKDOWN AND NOTHING MORE. IF HE GOT THAT IMPRESSION FROM ME, I AM TRULY SORRY.
