Narrative:

Aircraft Y was on approach at dbq on the ILS runway 31. Aircraft X was on the ground at dbq looking for departure clearance. I thought aircraft X was departing runway 31, so I gave aircraft X the departure clearance with the restr to fly runway heading, thinking to use the 45 degree rule. Dbq tower called and told me they were opening. I advised aircraft X was departing and I had aircraft Y on approach. The tower advised aircraft X was airborne. Satisfied that aircraft X was in the air, nwbound, I terminated radar on aircraft Y and put him on advisory frequency. I then saw an uncorrelated target 300 ft above aircraft Y, sebound. Aircraft X soon called and the computer acquired the target. That was when I realized aircraft X had departed runway 13, not runway 31. I informed my supervisor of the incident and it was investigated.

Title: ACFT X DEPARTED HEAD-ON INTO ARRIVING ACFT Y. RPTR THOUGHT ACFT X WAS DEPARTING OPPOSITE DIRECTION.

Narrative: ACFT Y WAS ON APCH AT DBQ ON THE ILS RWY 31. ACFT X WAS ON THE GND AT DBQ LOOKING FOR DEP CLRNC. I THOUGHT ACFT X WAS DEPARTING RWY 31, SO I GAVE ACFT X THE DEP CLRNC WITH THE RESTR TO FLY RWY HDG, THINKING TO USE THE 45 DEG RULE. DBQ TWR CALLED AND TOLD ME THEY WERE OPENING. I ADVISED ACFT X WAS DEPARTING AND I HAD ACFT Y ON APCH. THE TWR ADVISED ACFT X WAS AIRBORNE. SATISFIED THAT ACFT X WAS IN THE AIR, NWBOUND, I TERMINATED RADAR ON ACFT Y AND PUT HIM ON ADVISORY FREQ. I THEN SAW AN UNCORRELATED TARGET 300 FT ABOVE ACFT Y, SEBOUND. ACFT X SOON CALLED AND THE COMPUTER ACQUIRED THE TARGET. THAT WAS WHEN I REALIZED ACFT X HAD DEPARTED RWY 13, NOT RWY 31. I INFORMED MY SUPVR OF THE INCIDENT AND IT WAS INVESTIGATED.
