Narrative:

My maintenance logbook release qualification has been removed from the 'qualifications' section of my training records. Under 'training history' it shows that I had been trained and qualified on oct/xa/95. I have submitted the proper forms to have this put back into my records as 'qualified.' I have been releasing all types of aircraft for flight from 'OTS' to 'a' checks. This does not include ETOPS. I'm not sure when or why this was dropped. I was never notified.
