Narrative:

On may/xx/98 at XA18 local, flight xyz blocked out and departed for YYY. Upon arrival at YYY, the captain determined that we had left with an open write-up in the logbook. The crew oxygen gauge was placarded inoperative and required oxygen verification and signoff prior to each departure. In YYY a mechanic was summoned to fulfill the required check and document the work. Inadvertently, he marked through the ZZZ departure block (the open write-up) and placed YYY in its place, thereby showing a completed action from an incorrect airport.

Title: A B727 WAS DISPATCHED WITH A REQUIRED MEL SPECIAL PROC TO CHK THE CREW OXYGEN BOTTLE PRESSURE PRIOR TO EACH DEP NOT SIGNED FOR IN LOG.

Narrative: ON MAY/XX/98 AT XA18 LCL, FLT XYZ BLOCKED OUT AND DEPARTED FOR YYY. UPON ARR AT YYY, THE CAPT DETERMINED THAT WE HAD LEFT WITH AN OPEN WRITE-UP IN THE LOGBOOK. THE CREW OXYGEN GAUGE WAS PLACARDED INOP AND REQUIRED OXYGEN VERIFICATION AND SIGNOFF PRIOR TO EACH DEP. IN YYY A MECH WAS SUMMONED TO FULFILL THE REQUIRED CHK AND DOCUMENT THE WORK. INADVERTENTLY, HE MARKED THROUGH THE ZZZ DEP BLOCK (THE OPEN WRITE-UP) AND PLACED YYY IN ITS PLACE, THEREBY SHOWING A COMPLETED ACTION FROM AN INCORRECT ARPT.
