Narrative:

Flight plan after moony intersection direct czq using sjc 8 SID. The pre departure clearance showed both the moony 1 and sjc 8 departures (runways 12R and 30L) both sids go to moony intersection. After moony sjc 8 was planned direct czq. The moony 1 also goes to moony but was filed moony-pxn-czq. I looked at these and saw both went to a common intersection. For some reason I did not notice that the moony 1 has the added segment to pxn reflected in the pre departure clearance. The flight plan reflected the sjc 8 which did not include the pxn segment. This possibly added to my missing the pxn on the moony 1. Both sids have a pxn transition depicted. After takeoff and climbing through 12000 ft and after moony, we turned direct to czq. The controller asked where we were headed. We said czq. He gave us a 90 degree left turn then through approximately 12800 ft he gave us direct czq. In addition to me paying more attention to my work, I think each pre departure clearance should reflect 1 departure and not an either/or (runway 12R or runway 30L). Supplemental information from acn 398432: I set up the cockpit radios for the captain's departure and flight. Captain is still not at aircraft. The so advised me they were now using runway 12R instead of what the flight plan and SID had depicted to be runway 30L. I then set up the departure now for the moony 1 and proceeded to load the INS. Most of all the waypoints were not in the database and had to be loaded manually, which takes time. Still no captain at aircraft. Time is growing near to departure. The captain came in the cockpit, shuffled around and then took his seat. Note: in a prior conversation to the captain, I had asked him on the way to sjc if he had been in and out of sjc -- he said yes. I asked him also if he was familiar with the departures, and I said it could be difficult. He said yes. Now he is looking at the logbook and has a question which now I had to call operations to find out and send maintenance out and more time is ticking. I am aggravated because the captain had not arrived earlier. His tendency in the past has been to push it to the end. At this point, I will address the point that he has many personal problems. He is preoccupied, and basically his performance in an airplane has been lousy, slip-shod, not consistent, questionable in his judgement and awareness. I have spoken to him on several occasions about his old charts, sloppy job, and on our last flight he said he would sharpen up his performance, and knew he has grown lax. We get pushed. The INS waypoints are not crosschecked. I show him the pre departure clearance. He briefs heading, first radial, says he knows what he is doing. I then said we were given czq -- there must be a communication error. He said no problem. I then asked what was so important that he was late to the aircraft. He said he was talking to the caretaker at his house about something earlier today. I said this is not good, and that he needs to get his life together. I suggest calling in sick and take off a month.

Narrative: FLT PLAN AFTER MOONY INTXN DIRECT CZQ USING SJC 8 SID. THE PDC SHOWED BOTH THE MOONY 1 AND SJC 8 DEPS (RWYS 12R AND 30L) BOTH SIDS GO TO MOONY INTXN. AFTER MOONY SJC 8 WAS PLANNED DIRECT CZQ. THE MOONY 1 ALSO GOES TO MOONY BUT WAS FILED MOONY-PXN-CZQ. I LOOKED AT THESE AND SAW BOTH WENT TO A COMMON INTXN. FOR SOME REASON I DID NOT NOTICE THAT THE MOONY 1 HAS THE ADDED SEGMENT TO PXN REFLECTED IN THE PDC. THE FLT PLAN REFLECTED THE SJC 8 WHICH DID NOT INCLUDE THE PXN SEGMENT. THIS POSSIBLY ADDED TO MY MISSING THE PXN ON THE MOONY 1. BOTH SIDS HAVE A PXN TRANSITION DEPICTED. AFTER TKOF AND CLBING THROUGH 12000 FT AND AFTER MOONY, WE TURNED DIRECT TO CZQ. THE CTLR ASKED WHERE WE WERE HEADED. WE SAID CZQ. HE GAVE US A 90 DEG L TURN THEN THROUGH APPROX 12800 FT HE GAVE US DIRECT CZQ. IN ADDITION TO ME PAYING MORE ATTN TO MY WORK, I THINK EACH PDC SHOULD REFLECT 1 DEP AND NOT AN EITHER/OR (RWY 12R OR RWY 30L). SUPPLEMENTAL INFO FROM ACN 398432: I SET UP THE COCKPIT RADIOS FOR THE CAPT'S DEP AND FLT. CAPT IS STILL NOT AT ACFT. THE SO ADVISED ME THEY WERE NOW USING RWY 12R INSTEAD OF WHAT THE FLT PLAN AND SID HAD DEPICTED TO BE RWY 30L. I THEN SET UP THE DEP NOW FOR THE MOONY 1 AND PROCEEDED TO LOAD THE INS. I ASKED HIM ALSO IF HE WAS FAMILIAR WITH THE DEPS, AND I SAID IT COULD BE DIFFICULT. HE SAID YES. NOW HE IS LOOKING AT THE LOGBOOK AND HAS A QUESTION WHICH NOW I HAD TO CALL OPS TO FIND OUT AND SEND MAINT OUT AND MORE TIME IS TICKING. I AM AGGRAVATED BECAUSE THE CAPT HAD NOT ARRIVED EARLIER.
