Narrative:

An IFR flight plan was filed via telephone landline from tolson county (tso) to martinsburg, wv. My student's flight planning and routing were reviewed by me prior to her filing and everything was in order. We talked to a pilot who had just landed and he said that the tops were 6000 ft and that he had encountered no icing. She picked up her clearance by phone and was given a release time more than adequate to launch. We entered the clouds at approximately 2000 ft and initiated our call to pittsburgh approach rather than ZOB per the clearance. Once switched over to ZOB, they seemed quite perturbed since they had our flight plan out of harrison county which is a small airport approximately 20 mi south of carroll county. We were given a higher altitude of 6000 ft. At 4500 ft we began picking up slight traces of rime ice. At 5300 ft we were told to descend and maintain 5000 ft for traffic. Rather than take any chances, we amended our flight plan to wheeling (hlg) and got a clearance back down to 3000 ft. Prior to landing, center asked us to contact ZOB by landline to discuss the misfiling/miscom or misunderstanding. I spoke with a supervisor who said that he reviewed the tapes and that my student filed from harrison county. I spoke with her specifically about the possibility of her misfiling and she was clear that our departure airport was tolson county and that when we initiated communications with the center, they didn't even have our correct identify numbers. Somewhere along the line, someone misunderstood or misinterped. I am sure that a slight touch of the flu/pneumonia didn't put me in the greatest shape to fly but then I only found out about my condition upon getting back home to maryland. One last item: upon our descent en route to belair VOR, we were cleared to 3000 ft and dropped down to 2700 ft momentarily due to carburetor icing. The controller said we should have reported this change of altitude but we had our hands full just trying to clear up the problem.

RATHER THAN TAKE ANY CHANCES, WE AMENDED OUR FLT PLAN TO WHEELING (HLG) AND GOT A CLRNC BACK DOWN TO 3000 FT. PRIOR TO LNDG, CTR ASKED US TO CONTACT ZOB BY LANDLINE TO DISCUSS THE MISFILING/MISCOM OR MISUNDERSTANDING. I SPOKE WITH A SUPVR WHO SAID THAT HE REVIEWED THE TAPES AND THAT MY STUDENT FILED FROM HARRISON COUNTY. I SPOKE WITH HER SPECIFICALLY ABOUT THE POSSIBILITY OF HER MISFILING AND SHE WAS CLR THAT OUR DEP ARPT WAS TOLSON COUNTY AND THAT WHEN WE INITIATED COMS WITH THE CTR, THEY DIDN'T EVEN HAVE OUR CORRECT IDENT NUMBERS. SOMEWHERE ALONG THE LINE, SOMEONE MISUNDERSTOOD OR MISINTERPED. I AM SURE THAT A SLIGHT TOUCH OF THE FLU/PNEUMONIA DIDN'T PUT ME IN THE GREATEST SHAPE TO FLY BUT THEN I ONLY FOUND OUT ABOUT MY CONDITION UPON GETTING BACK HOME TO MARYLAND. ONE LAST ITEM: UPON OUR DSCNT ENRTE TO BELAIR VOR, WE WERE CLRED TO 3000 FT AND DROPPED DOWN TO 2700 FT MOMENTARILY DUE TO CARB ICING. THE CTLR SAID WE SHOULD HAVE RPTED THIS CHANGE OF ALT BUT WE HAD OUR HANDS FULL JUST TRYING TO CLR UP THE PROB.
