Narrative:

Report summary: after surgery, I took an FAA flight physical and passed for a third class certificate. I gathered all the required documents and sent them to the FAA. Apparently the FAA office lost the documents but I got copies and resubmitted. After numerous calls for follow up action taken, I continued to be stalled, plus had to get more documents. Months later, I still had not received my license or certificate but was billed by an FAA doctor. I feel the entire handling by the FAA is insulting.
