Narrative:

We departed on IFR flight plan from leb on runway 36. SID says 'climb runway heading to 2400 ft before proceeding on course.' leb tower cleared us for takeoff and gave us 4000 ft as an initial altitude. Once airborne we were handed off to boston. Neither myself nor the captain heard leb tower clear us on course, so we proceeded runway heading and climbed while checking on with boston. Once in radar contact, bos informed us that we were way north of course and should turn immediately on course. Airspace below 4000-6000 ft is non radar in that area. I feel better communication could have avoided this problem. IFR departure procedure for runway 36 was reviewed, however, a good note to add for such a review would be: once reaching 2400 ft should we expect ATC to clear us on course? Should we query ATC before proceeding on course? I think such a discussion between crew members during IFR departure procedure review would have helped avoid the misunderstanding and ensuing course deviation. A query of ATC at 2400 ft about proceeding on course would have also helped.
