Narrative:

During the early morning of sep/xx/97, I was performing maintenance on aircraft XXX right engine. I noticed the exhaust fairing had an access panel missing. I checked on the part and was issued it from parts. I installed it per the maintenance manual. After coming down from the top of the engine I was assigned to finish the service check inspection and other scheduled maintenance. The access panel installed was not a complete assembly, but was installed per the maintenance manual instructions. This panel missing was not entered into the aircraft logbook due to my being assigned other tasks. This happened because the aircraft arrived at the hangar approximately 2 1/2 hours after our shift started and we were trying to complete the scheduled tasks. Aircraft came into hangar, possibly for scheduled maintenance, the next evening and the access panel was found bent and sticking out. The engine tailpipe fairing was replaced due to dents on it. These dents were not from this panel.

Title: A CANADAIR RJ INCURRED DAMAGE TO THE #2 ENG THRUST REVERSER FAIRING CAUSED BY AN IMPROPERLY INSTALLED FAIRING PANEL.

Narrative: DURING THE EARLY MORNING OF SEP/XX/97, I WAS PERFORMING MAINT ON ACFT XXX R ENG. I NOTICED THE EXHAUST FAIRING HAD AN ACCESS PANEL MISSING. I CHKED ON THE PART AND WAS ISSUED IT FROM PARTS. I INSTALLED IT PER THE MAINT MANUAL. AFTER COMING DOWN FROM THE TOP OF THE ENG I WAS ASSIGNED TO FINISH THE SVC CHK INSPECTION AND OTHER SCHEDULED MAINT. THE ACCESS PANEL INSTALLED WAS NOT A COMPLETE ASSEMBLY, BUT WAS INSTALLED PER THE MAINT MANUAL INSTRUCTIONS. THIS PANEL MISSING WAS NOT ENTERED INTO THE ACFT LOGBOOK DUE TO MY BEING ASSIGNED OTHER TASKS. THIS HAPPENED BECAUSE THE ACFT ARRIVED AT THE HANGAR APPROX 2 1/2 HRS AFTER OUR SHIFT STARTED AND WE WERE TRYING TO COMPLETE THE SCHEDULED TASKS. ACFT CAME INTO HANGAR, POSSIBLY FOR SCHEDULED MAINT, THE NEXT EVENING AND THE ACCESS PANEL WAS FOUND BENT AND STICKING OUT. THE ENG TAILPIPE FAIRING WAS REPLACED DUE TO DENTS ON IT. THESE DENTS WERE NOT FROM THIS PANEL.
