Narrative:

This event occurred on the third night of the trip. We had been on duty about 7 hours. The aircraft we were taking outbound was late arriving. After our preflight we were waiting in the aircraft for the loading to be completed. Once the loading was done, we were told by maintenance they could not find the lock bars for the cargo containers. We were becoming more fatigued as we waited. When we received our weight and balance information, we computed our takeoff data and departed. Once en route, we were called by the company and told our jump seater had been left off the weight and balance, and that would put us out of center of gravity limits for our flight. I believe that our delay and crew fatigue were the major factors in missing the jump seater not being on the weight and balance.

Title: FLC OF A B727 MISSED SHOWING THE JUMP SEATER ON THE ACFT WT AND BAL DURING PREFLT RESULTING IN THE ACFT CTR OF GRAVITY BEING OUT OF LIMITS.

Narrative: THIS EVENT OCCURRED ON THE THIRD NIGHT OF THE TRIP. WE HAD BEEN ON DUTY ABOUT 7 HRS. THE ACFT WE WERE TAKING OUTBOUND WAS LATE ARRIVING. AFTER OUR PREFLT WE WERE WAITING IN THE ACFT FOR THE LOADING TO BE COMPLETED. ONCE THE LOADING WAS DONE, WE WERE TOLD BY MAINT THEY COULD NOT FIND THE LOCK BARS FOR THE CARGO CONTAINERS. WE WERE BECOMING MORE FATIGUED AS WE WAITED. WHEN WE RECEIVED OUR WT AND BAL INFO, WE COMPUTED OUR TKOF DATA AND DEPARTED. ONCE ENRTE, WE WERE CALLED BY THE COMPANY AND TOLD OUR JUMP SEATER HAD BEEN LEFT OFF THE WT AND BAL, AND THAT WOULD PUT US OUT OF CTR OF GRAVITY LIMITS FOR OUR FLT. I BELIEVE THAT OUR DELAY AND CREW FATIGUE WERE THE MAJOR FACTORS IN MISSING THE JUMP SEATER NOT BEING ON THE WT AND BAL.
