Narrative:

We entered the cuban fir without contacting cuban control. Reason: fatigue, plain and simple. I was so tired I could hardly stay awake. My required duty rest was interrupted the night before by company. The previous night I had a 14 hour duty day and this day turned into a 12 hour day. This could be fixed by putting some sort of limits on international duty time limits.

Title: CREW OF DC8 FREIGHTER FAILS TO CONTACT MUHA CTL FOR ENTRY INTO THEIR FLT INFO REGION (FIR). FO RPTR CITES ACR POLICY THAT INTERRUPTS THEIR SLEEP PATTERN AND CREATED FATIGUE, THUS LOSING SITUATIONAL AWARENESS.

Narrative: WE ENTERED THE CUBAN FIR WITHOUT CONTACTING CUBAN CTL. REASON: FATIGUE, PLAIN AND SIMPLE. I WAS SO TIRED I COULD HARDLY STAY AWAKE. MY REQUIRED DUTY REST WAS INTERRUPTED THE NIGHT BEFORE BY COMPANY. THE PREVIOUS NIGHT I HAD A 14 HR DUTY DAY AND THIS DAY TURNED INTO A 12 HR DAY. THIS COULD BE FIXED BY PUTTING SOME SORT OF LIMITS ON INTL DUTY TIME LIMITS.
