Narrative:

Gear box was installed on this engine in ZZZ. The engine was installed on aircraft in YYY. Engine failed first flight out of YYY (passing through FL250 in climb). Engine shop suspects oil tube between accessory gear box engine was (is) obstructed. Engine has not been disassembled at this time. This NASA form is being filed in case there was an error when accessory gear box was installed.
