Narrative:

Right elephant ear missing, with possible takeoff and landing weight restrs plus additional burn required was never given to dispatcher ZZZ prior to his flight a, phl-bos and flight B, bos-mco departing. Aircraft number was B737-300 XXX. When tech man abc was questioned why it wasn't given, he said it was given verbally to previous shift dispatcher and put in computer at XA01. Never came off computer release.

Title: A B737-300 WAS DISPATCHED WITH AN INCORRECT FLT PLAN OMITTING EXTRA FUEL BURN AND LNDG WT RESTRS DUE TO A REMOVED FLAP ACTUATOR FAIRING.

Narrative: R ELEPHANT EAR MISSING, WITH POSSIBLE TKOF AND LNDG WT RESTRS PLUS ADDITIONAL BURN REQUIRED WAS NEVER GIVEN TO DISPATCHER ZZZ PRIOR TO HIS FLT A, PHL-BOS AND FLT B, BOS-MCO DEPARTING. ACFT NUMBER WAS B737-300 XXX. WHEN TECH MAN ABC WAS QUESTIONED WHY IT WASN'T GIVEN, HE SAID IT WAS GIVEN VERBALLY TO PREVIOUS SHIFT DISPATCHER AND PUT IN COMPUTER AT XA01. NEVER CAME OFF COMPUTER RELEASE.
