Narrative:

I found unlatched fasteners under the right engine during my exterior inspection. Because of maintenance cards and forms in the logbook, the captain said it looked as if maintenance was still working the aircraft. I informed him of the latches. Approximately 10 mins prior to departure a maintenance man arrived and began to sign the logbook. I asked what he had done and had he left the latches open for his coffee break. The captain asked if the latches were secure to which the maintenance man nodded affirmatively. As gear locked up on takeoff, tower said we had a flapping right cowling. Returned to runway 9R mia. Overweight landing and cowling departed aircraft at touchdown. Turned off runway and shut down right engine for inspection. Returned to gate.

Title: A B757 WAS DISPATCHED WITH THE R ENG COWLING NOT SECURED. ACFT RETURNED TO THE FIELD AND ON LNDG THE COWLING FELL OFF.

Narrative: I FOUND UNLATCHED FASTENERS UNDER THE R ENG DURING MY EXTERIOR INSPECTION. BECAUSE OF MAINT CARDS AND FORMS IN THE LOGBOOK, THE CAPT SAID IT LOOKED AS IF MAINT WAS STILL WORKING THE ACFT. I INFORMED HIM OF THE LATCHES. APPROX 10 MINS PRIOR TO DEP A MAINT MAN ARRIVED AND BEGAN TO SIGN THE LOGBOOK. I ASKED WHAT HE HAD DONE AND HAD HE LEFT THE LATCHES OPEN FOR HIS COFFEE BREAK. THE CAPT ASKED IF THE LATCHES WERE SECURE TO WHICH THE MAINT MAN NODDED AFFIRMATIVELY. AS GEAR LOCKED UP ON TKOF, TWR SAID WE HAD A FLAPPING R COWLING. RETURNED TO RWY 9R MIA. OVERWT LNDG AND COWLING DEPARTED ACFT AT TOUCHDOWN. TURNED OFF RWY AND SHUT DOWN R ENG FOR INSPECTION. RETURNED TO GATE.
