Narrative:

On preflight, equipment door light would not extinguish with door verified closed. The mechanic deferred the door light. Prior to pushback, we twice verified with mechanic that door was closed. On takeoff, the aircraft would not pressurize and very noisy -- door was apparently open. We returned for an overweight landing.

Title: AN LGT RETURNED TO THE FIELD IMMEDIATELY AFTER TKOF DUE TO UNABLE TO PRESSURIZE THE ACFT. FOUND EQUIP ACCESS DOOR OPEN AND DOOR WARNING SYS PREVIOUSLY DEFERRED.

Narrative: ON PREFLT, EQUIP DOOR LIGHT WOULD NOT EXTINGUISH WITH DOOR VERIFIED CLOSED. THE MECH DEFERRED THE DOOR LIGHT. PRIOR TO PUSHBACK, WE TWICE VERIFIED WITH MECH THAT DOOR WAS CLOSED.
