Narrative:

On sep/mon/96 I was informed of the duty log deficiency on dec/tue/95. That this unusual entry in my flight time/duty log was not discovered until a record inspection on sep/wed/96 by mr X, pio ZZZ FSDO. My contention is that the duty log entry on that date was the result of my inattn to detail in logging my actual flight and rest time for the duty period in question. To the contrary to what I recorded on that date, I believe that I did receive 10 hours of rest before reporting for duty at XG00. This is not the first time I have had problems with this new 'trucker's log' format, and it is very easy to incorrectly mark the duty, flight, or rest block. I have no reason to ever violate the 10 hour rest requirement, this is something you have ingrained in us as an EMS pilot. As indicated on the flight manifest on dec/tue/95, I departed on a patient flight at XG29 that morning and landed at 'a' hospital at XG49. I departed 'a' hospital at XH01. I encountered subminimum WX at 'B' hospital and at the airport, so I elected to divert to 'C' hospital and landed at XH20, where the patient was transported by ground ambulance to 'B' hospital with the flight nurse and flight paramedic. I stayed with the aircraft at the hospital. Mr Y, the relief pilot, upon learning that I landed at 'C' hospital, drove my truck to 'C' hospital, for I left my keys on the desk knowing that the WX (fog) could change before my return. I do not recall the precise time when mr Y arrived at the hospital, but I only live about 5 mins from 'C' hospital. I actually meant to log a block of flight time from XG30 to XH30, and duty time to around XH30 to XH45. I did not have a nurse or paramedic nor the flying WX to continue to log any additional duty time. I did not complete my flight/duty log until I arrived for duty at XS00, and that is when I made an unusual entry. If I have failed, it's correctly logging my flight and duty time on the day in question on this hard to read 'trucker's log,' and I will make every effort that this error will never happen again.

THIS IS NOT THE FIRST TIME I HAVE HAD PROBS WITH THIS NEW 'TRUCKER'S LOG' FORMAT, AND IT IS VERY EASY TO INCORRECTLY MARK THE DUTY, FLT, OR REST BLOCK. I HAVE NO REASON TO EVER VIOLATE THE 10 HR REST REQUIREMENT, THIS IS SOMETHING YOU HAVE INGRAINED IN US AS AN EMS PLT. AS INDICATED ON THE FLT MANIFEST ON DEC/TUE/95, I DEPARTED ON A PATIENT FLT AT XG29 THAT MORNING AND LANDED AT 'A' HOSPITAL AT XG49. I DEPARTED 'A' HOSPITAL AT XH01. I ACTUALLY MEANT TO LOG A BLOCK OF FLT TIME FROM XG30 TO XH30, AND DUTY TIME TO AROUND XH30 TO XH45. I DID NOT HAVE A NURSE OR PARAMEDIC NOR THE FLYING WX TO CONTINUE TO LOG ANY ADDITIONAL DUTY TIME. I DID NOT COMPLETE MY FLT/DUTY LOG UNTIL I ARRIVED FOR DUTY AT XS00, AND THAT IS WHEN I MADE AN UNUSUAL ENTRY. IF I HAVE FAILED, IT'S CORRECTLY LOGGING MY FLT AND DUTY TIME ON THE DAY IN QUESTION ON THIS HARD TO READ 'TRUCKER'S LOG,' AND I WILL MAKE EVERY EFFORT THAT THIS ERROR WILL NEVER HAPPEN AGAIN.
