Narrative:

XB15. At 20 mi south of lakefront airport, contacted new orleans approach, was issued a transponder code, and cleared into the class B airspace at 2500 ft. Informed new orleans approach that aircraft was VFR to lakefront airport with current ATIS information. Informed grand isle tower of positive control with new orleans approach. Closed flight plan with grand isle to reflect landing time at lakefront airport of XB25. Tuned second VHF radio to lakefront tower frequency to monitor traffic. XB20 local. At 8.7 mi south of lakefront airport, was contacted by new orleans approach with updated information on traffic, and tower frequency for lakefront airport. New orleans approach requested contact when lakefront airport was in sight. Reported to new orleans approach that lakefront airport was already in sight. At 8.5 mi from lakefront airport, was instructed by new orleans approach to contact lakefront tower. Contacted lakefront tower with position report, intentions, and ATIS information. Was instructed to expect landing on taxiway F right of runway 36L, and report 2 mi out. Confirmed instructions, requested and received permission to start descent from 2500 ft down to 500 ft. XB22. After descending from 2500 ft to 1800 ft using a descent rate of approximately 1000 FPM, was informed to follow bonanza that was turning to final approach at 1500 ft. Bonanza could not be located. The following sequence of events took place in less than 10 seconds. TCAS audio reported 'warning aircraft less than 1 mi.' check of TCAS display showed numerous targets. Passenger pointed to the left side of aircraft. Bonanza still not visible. Informed lakefront tower of intentions to execute a right 360 degree standard rate turn for spacing. Lakefront tower acknowledged intentions. Ground speed indicated 127 KTS. Checked right side of aircraft to clear for spacing turn. While scanning instruments prior to executing spacing turn, bonanza traveling on same heading descended from overhead with 15 ft of main rotor blades with gear up. Contacted lakefront tower with the following statement: 'that was closer than I thought!' the bonanza reported he lost sight of the helicopter in the turn. Spacing turn was never executed. Lakefront tower informed another aircraft to follow the helicopter in on approach. XB25. Landed at lakefront without further incident. [Tower supervisor's comments]: 'after reviewing the tapes of lakefront tower for the saturday time frame, it appears the helicopter was in error.' 'all instructions given to the helicopter were based on baron traffic.' at this point I read my events details to tower supervisor starting at the SB20 local paragraph and reading until the end. After hearing the events, tower supervisor made the following observations: tower supervisor said 'the traffic information relayed to you by lakefront tower was about a baron on extended left base to runway 36L.' 'his position was reported at 1500 ft and in front of you.' 'there was a bonanza on short final to runway 36L.' 'the bonanza was the aircraft that you saw below, and in front of you.' 'believing the bonanza was the baron you were supposed to follow, you gave a positive contact, and continued your approach.' 'a second bonanza was behind you, followed by a mooney.' 'these aircraft were not a factor, but added to the confusion.' 'similar types of aircraft different locations in sequencing.' 'at this point, events continued as you made your approach, and the baron passed you from overhead.' 'also, lakefront tower addressed your aircraft call sign axyz as bxyz, axyz, and cxyz.' 'this also added to the confusion.' 'at this time, lakefront tower views this as a non-event.'

AT THIS POINT I READ MY EVENTS DETAILS TO TWR SUPVR STARTING AT THE SB20 LCL PARAGRAPH AND READING UNTIL THE END. AFTER HEARING THE EVENTS, TWR SUPVR MADE THE FOLLOWING OBSERVATIONS: TWR SUPVR SAID 'THE TFC INFO RELAYED TO YOU BY LAKEFRONT TWR WAS ABOUT A BARON ON EXTENDED L BASE TO RWY 36L.' 'HIS POS WAS RPTED AT 1500 FT AND IN FRONT OF YOU.' 'THERE WAS A BONANZA ON SHORT FINAL TO RWY 36L.' 'THE BONANZA WAS THE ACFT THAT YOU SAW BELOW, AND IN FRONT OF YOU.' 'BELIEVING THE BONANZA WAS THE BARON YOU WERE SUPPOSED TO FOLLOW, YOU GAVE A POSITIVE CONTACT, AND CONTINUED YOUR APCH.' 'A SECOND BONANZA WAS BEHIND YOU, FOLLOWED BY A MOONEY.' 'THESE ACFT WERE NOT A FACTOR, BUT ADDED TO THE CONFUSION.' 'SIMILAR TYPES OF ACFT DIFFERENT LOCATIONS IN SEQUENCING.'
