Narrative:

We took off on runway 31C out of mdw airport. Our clearance was to climb to 3000 ft, with a right turn to 090 degrees. When we called departure he said we were only cleared to 2000 ft, but climb and maintain 3000 ft. He then stated and we also repeated -- give me a good rate through 3000 ft, climb and maintain 12000 ft. He then stopped us at 4000 ft wanting to know what we were doing. We stayed at 4000 ft. Traffic went over us at 5000 ft. Major miscom contributed to this starting from departure not talking to the tower about what they needed. All our 'wrong' readbacks were missed also.

Title: MISCOM BTWN ATC AND FLC. EACH TIME ALT CLRNC WAS GIVEN, FLC READ IT BACK. ATC QUESTIONED FLC WHEN CLBING THROUGH THE 'ASSIGNED,' BUT FLC HAD READ BACK THE CLRNC PRIOR.

TFC WENT OVER US AT 5000 FT. MAJOR MISCOM CONTRIBUTED TO THIS STARTING FROM DEP NOT TALKING TO THE TWR ABOUT WHAT THEY NEEDED. ALL OUR 'WRONG' READBACKS WERE MISSED ALSO.
