Narrative:

We were on an ILS in VMC conditions and asked to keep our speed up. We had just left icing conditions earlier and had some residual ice remaining. We were told to contact tower over the OM. At this time, when we began to configure for landing and selected flaps we got a flap warning fault due to a split on the flap position indicator and were unable to use flaps. This required an increase of 40 KTS to our reference speed and last checks for final. With this occurring we forgot to change frequency and receive landing clearance. It was light traffic at cle and no traffic conflicts took place. We were told on the ground by the tower people that all was ok and there were no problems with them and to have a nice day.

Title: FLC OF AN LTT FORGOT TO CONTACT THE TWR FOR LNDG CLRNC DUE TO ACFT EQUIP (FLAPS) MALFUNCTION.

Narrative: WE WERE ON AN ILS IN VMC CONDITIONS AND ASKED TO KEEP OUR SPD UP. WE HAD JUST LEFT ICING CONDITIONS EARLIER AND HAD SOME RESIDUAL ICE REMAINING. WE WERE TOLD TO CONTACT TWR OVER THE OM. AT THIS TIME, WHEN WE BEGAN TO CONFIGURE FOR LNDG AND SELECTED FLAPS WE GOT A FLAP WARNING FAULT DUE TO A SPLIT ON THE FLAP POS INDICATOR AND WERE UNABLE TO USE FLAPS. THIS REQUIRED AN INCREASE OF 40 KTS TO OUR REF SPD AND LAST CHKS FOR FINAL. WITH THIS OCCURRING WE FORGOT TO CHANGE FREQ AND RECEIVE LNDG CLRNC. IT WAS LIGHT TFC AT CLE AND NO TFC CONFLICTS TOOK PLACE. WE WERE TOLD ON THE GND BY THE TWR PEOPLE THAT ALL WAS OK AND THERE WERE NO PROBS WITH THEM AND TO HAVE A NICE DAY.
