Narrative:

Departing dfw we pushed back from gate with tow to gate X for start. Lot of saab traffic on ramp. Clear to start engine normal start. Went to start. First officer remarked about saab traffic on left. Ground crew called for park, I cleared them. First officer was looking in kit bag. I noticed engine start switches were still on. I thought it odd I did not turn them off after engine start. I now believed that engine #2 was running. First officer had punched out start annunciator panel. We taxied to runway 17R, got in line, aircraft was light so the pulling to right I thought could be wind or just tracking. We were cleared into position, cleared for takeoff. As I add power the EPR on engine #2 was not coming up. We looked at the engine again and found it was shut down. I immediately cleared the runway and started engine #2. All taxi checklists had been performed normally. With the start annunciator panel not in backup there is no other engine out indication on taxi, other than my not seeing an engine not running. The rest of the flight was normal. In about 10000 hours of flying I would have never believed I could do this. I got distracted by traffic, the other things going on around our jobs, and failed to lead when the first officer wanted to turn out the start annunciator panel which is now procedure.

Title: FLC FAILED TO COMPLETE ENG START ON ONE OF TWO ENGS PRIOR TO ATTEMPTING TKOF.

I NOW BELIEVED THAT ENG #2 WAS RUNNING. FO HAD PUNCHED OUT START ANNUNCIATOR PANEL. WE TAXIED TO RWY 17R, GOT IN LINE, ACFT WAS LIGHT SO THE PULLING TO R I THOUGHT COULD BE WIND OR JUST TRACKING. WE WERE CLRED INTO POS, CLRED FOR TKOF. AS I ADD PWR THE EPR ON ENG #2 WAS NOT COMING UP. WE LOOKED AT THE ENG AGAIN AND FOUND IT WAS SHUT DOWN. I IMMEDIATELY CLRED THE RWY AND STARTED ENG #2. ALL TAXI CHKLISTS HAD BEEN PERFORMED NORMALLY. WITH THE START ANNUNCIATOR PANEL NOT IN BACKUP THERE IS NO OTHER ENG OUT INDICATION ON TAXI, OTHER THAN MY NOT SEEING AN ENG NOT RUNNING. THE REST OF THE FLT WAS NORMAL. IN ABOUT 10000 HRS OF FLYING I WOULD HAVE NEVER BELIEVED I COULD DO THIS. I GOT DISTRACTED BY TFC, THE OTHER THINGS GOING ON AROUND OUR JOBS, AND FAILED TO LEAD WHEN THE FO WANTED TO TURN OUT THE START ANNUNCIATOR PANEL WHICH IS NOW PROC.
