Narrative:

On sep/sat/95 airlines flight DC10 arrived at dtw from sfo. Landed on runway 21R and exited the runway at taxiway Y9. Ground controller gave us the option to taxi to parking via taxiway Y or taxiway K. The captain elected to make the hard left turn onto taxiway Y. While taxiing northeast on Y the ground controller notified us that we had run over a taxiway light at the intersection of Y9 and Y. Postflt inspection by the so revealed numerous pieces of blue glass imbedded in the forward outer left main gear tire. A logbook entry was made and local maintenance was called. The damage to the tire required replacing the tire. I understand that running over a taxiway light at this location has happened numerous times. Perhaps more concrete at this intersection with relocated taxiway lights would help the situation. A note of caution in the taxiway plates (especially for widebody transport's) would also help prevent a recurrence of this problem. A brief note in the company safety publication could help alert pilots to be extra cautious when exiting the runway at this taxiway.

Title: TXWY EXCURSION, RAN OVER TAXI LIGHT.

Narrative: ON SEP/SAT/95 AIRLINES FLT DC10 ARRIVED AT DTW FROM SFO. LANDED ON RWY 21R AND EXITED THE RWY AT TXWY Y9. GND CTLR GAVE US THE OPTION TO TAXI TO PARKING VIA TXWY Y OR TXWY K. THE CAPT ELECTED TO MAKE THE HARD L TURN ONTO TXWY Y. WHILE TAXIING NE ON Y THE GND CTLR NOTIFIED US THAT WE HAD RUN OVER A TXWY LIGHT AT THE INTXN OF Y9 AND Y. POSTFLT INSPECTION BY THE SO REVEALED NUMEROUS PIECES OF BLUE GLASS IMBEDDED IN THE FORWARD OUTER L MAIN GEAR TIRE. A LOGBOOK ENTRY WAS MADE AND LCL MAINT WAS CALLED. THE DAMAGE TO THE TIRE REQUIRED REPLACING THE TIRE. I UNDERSTAND THAT RUNNING OVER A TXWY LIGHT AT THIS LOCATION HAS HAPPENED NUMEROUS TIMES. PERHAPS MORE CONCRETE AT THIS INTXN WITH RELOCATED TXWY LIGHTS WOULD HELP THE SIT. A NOTE OF CAUTION IN THE TXWY PLATES (ESPECIALLY FOR WDB'S) WOULD ALSO HELP PREVENT A RECURRENCE OF THIS PROB. A BRIEF NOTE IN THE COMPANY SAFETY PUB COULD HELP ALERT PLTS TO BE EXTRA CAUTIOUS WHEN EXITING THE RWY AT THIS TXWY.
