Narrative:

Jfk clearance changed filed flight plan adding waypoint linnd between shipp and champ. After passing shipp we were given 'heading 140 degrees, vectors for champ.' next controller issued new heading and clearance via linnd. Controller then asked whether we had previously received clearance via linnd. I stated that our clearance was over shipp to champ, not realizing that the controller was really asking whether we had received an amended clearance prior to flight. In addition, the intersection linnd did not register since I had failed to reprogram the INS. I quickly informed ATC that I was mistaken. There were no traffic conflicts. Contributing factors: 1) pressure for 'on time' departure, 2) inexperienced first officer, 3) poor use of CRM by me (I rushed first officer), 4) had problems programming our INS system (CDU would not accept latitude and longitude and introduced errors into other 2 units).

Title: FLC FAILED TO PROGRAM AN INERTIAL FIX IN AN OVERWATER OP.

Narrative: JFK CLRNC CHANGED FILED FLT PLAN ADDING WAYPOINT LINND BTWN SHIPP AND CHAMP. AFTER PASSING SHIPP WE WERE GIVEN 'HDG 140 DEGS, VECTORS FOR CHAMP.' NEXT CTLR ISSUED NEW HDG AND CLRNC VIA LINND. CTLR THEN ASKED WHETHER WE HAD PREVIOUSLY RECEIVED CLRNC VIA LINND. I STATED THAT OUR CLRNC WAS OVER SHIPP TO CHAMP, NOT REALIZING THAT THE CTLR WAS REALLY ASKING WHETHER WE HAD RECEIVED AN AMENDED CLRNC PRIOR TO FLT. IN ADDITION, THE INTXN LINND DID NOT REGISTER SINCE I HAD FAILED TO REPROGRAM THE INS. I QUICKLY INFORMED ATC THAT I WAS MISTAKEN. THERE WERE NO TFC CONFLICTS. CONTRIBUTING FACTORS: 1) PRESSURE FOR 'ON TIME' DEP, 2) INEXPERIENCED FO, 3) POOR USE OF CRM BY ME (I RUSHED FO), 4) HAD PROBS PROGRAMMING OUR INS SYS (CDU WOULD NOT ACCEPT LATITUDE AND LONGITUDE AND INTRODUCED ERRORS INTO OTHER 2 UNITS).
