Narrative:

We preflted the aircraft and noted that seat xx was MEL'ed as seat cushion was not installed due to previous one was soiled. The captain also noted that there was a cushion in the seat and it seemed to be the proper one. The MEL was also listed on the release. After departure we noted that there was a full complement of passenger in first class, although the cabin was not full. During taxi out it had not registered that the first class was full because the cabin was not full. Also because the seat had a cushion we had not made a strong mental note of it. Also the placard required to be on the seat of 'do not use' was missing. When the weight and balance comes we are busy with many checks and outside duties, and this one slipped by unnoticed as we did not have a full flight. Also because of the missing placard the flight attendants missed this one also.

Title: DURING PREFLT THE FLC FAILED TO ASSURE THAT AN MEL'ED PAX SEAT WAS PLACARDED FOR NON USE AND NOT USED.

Narrative: WE PREFLTED THE ACFT AND NOTED THAT SEAT XX WAS MEL'ED AS SEAT CUSHION WAS NOT INSTALLED DUE TO PREVIOUS ONE WAS SOILED. THE CAPT ALSO NOTED THAT THERE WAS A CUSHION IN THE SEAT AND IT SEEMED TO BE THE PROPER ONE. THE MEL WAS ALSO LISTED ON THE RELEASE. AFTER DEP WE NOTED THAT THERE WAS A FULL COMPLEMENT OF PAX IN FIRST CLASS, ALTHOUGH THE CABIN WAS NOT FULL. DURING TAXI OUT IT HAD NOT REGISTERED THAT THE FIRST CLASS WAS FULL BECAUSE THE CABIN WAS NOT FULL. ALSO BECAUSE THE SEAT HAD A CUSHION WE HAD NOT MADE A STRONG MENTAL NOTE OF IT. ALSO THE PLACARD REQUIRED TO BE ON THE SEAT OF 'DO NOT USE' WAS MISSING. WHEN THE WT AND BAL COMES WE ARE BUSY WITH MANY CHKS AND OUTSIDE DUTIES, AND THIS ONE SLIPPED BY UNNOTICED AS WE DID NOT HAVE A FULL FLT. ALSO BECAUSE OF THE MISSING PLACARD THE FLT ATTENDANTS MISSED THIS ONE ALSO.
