Narrative:
Incorrectly told PF that clearance was a frees 5 instead of a santa rosa 5 departure. A left turn off of runway 19 direct to 'snupy' intersection instead of a right turn was discovered too late to rectify. WX was clear and 40 mi, our TCASII showed no traffic. The problem was caused by reading the wrong departure instructions. A contributing factor was the commercial chart page flipped over to reveal a '5' departure. The 'frees 5' instead of 'santa rosa 5' wasn't caught until too late in the turn. Trying to find 'snupy' on the frees 5 was how I discovered the mistake.
Original NASA ASRS Text
Title: DEV FROM CLRNC RTE. CPR CREW FLEW THE WRONG DEP.
Narrative: INCORRECTLY TOLD PF THAT CLRNC WAS A FREES 5 INSTEAD OF A SANTA ROSA 5 DEP. A L TURN OFF OF RWY 19 DIRECT TO 'SNUPY' INTXN INSTEAD OF A R TURN WAS DISCOVERED TOO LATE TO RECTIFY. WX WAS CLR AND 40 MI, OUR TCASII SHOWED NO TFC. THE PROB WAS CAUSED BY READING THE WRONG DEP INSTRUCTIONS. A CONTRIBUTING FACTOR WAS THE COMMERCIAL CHART PAGE FLIPPED OVER TO REVEAL A '5' DEP. THE 'FREES 5' INSTEAD OF 'SANTA ROSA 5' WASN'T CAUGHT UNTIL TOO LATE IN THE TURN. TRYING TO FIND 'SNUPY' ON THE FREES 5 WAS HOW I DISCOVERED THE MISTAKE.
