Narrative:

After departing mxp on a banko departure, after FL100 (T altitude 6000 ft) I began my flight engineer departure duties and was not actively monitoring the flight path of the aircraft until I noticed a discussion between the pilots about being unable to contact ATC (on a new frequency) due to frequency congestion to obtain a higher altitude. We had just crossed ometo intersection (42 DME srn VOR 278 degree radial) at FL120. We continued on course into the mountainous terrain 6000 ft below the minimum crossing altitude of FL180. We were on an IFR flight plan in VMC conditions. We were 14 mi (56 DME) west of ometo intersection before we received an urgent clearance from ATC to climb to FL260. ATC was upset at our altitude and location and repeatedly requested our flying conditions (VMC). We were flying through valleys into rising terrain and with terrain above the aircraft to our right (5 mi?) during our subsequent climb to FL260, I examined the pilot's banko departure page and then realized how low we really were on the departure profile. A 'new hire' captain was flying left seat on his IOE. A check captain was flying in the right seat working the radios. No comments were made by either pilot as to why we proceeded west of ometo intersection so far below the minimum crossing altitude on the departure profile.
