Narrative:

Working as a departure radar controller at bay TRACON, I was working runway 11 departures off of oak. Air carrier X departed on the wrong transponder code. ARTS data tag did not acquire on the target. During a high workload situation I didn't notice the lack of the data block and forgot air carrier X was on frequency and in my airspace. He entered center airspace without a handoff. Tower didn't advise me that the data block didn't acquire as required by the LOA and air carrier X on the wrong code plus my lack of awareness contributed to the error.

Narrative: WORKING AS A DEP RADAR CTLR AT BAY TRACON, I WAS WORKING RWY 11 DEPS OFF OF OAK. ACR X DEPARTED ON THE WRONG XPONDER CODE. ARTS DATA TAG DID NOT ACQUIRE ON THE TARGET. TWR DIDN'T ADVISE ME THAT THE DATA BLOCK DIDN'T ACQUIRE AS REQUIRED BY THE LOA AND ACR X ON THE WRONG CODE PLUS MY LACK OF AWARENESS CONTRIBUTED TO THE ERROR.
