Narrative:
I am being wrongly accused as the primary controller at fault in this situation. Due to the inadequacies of the radar controller, who failed to separate these aircraft, our quality assurance office at this time is holding me responsible. I will be requesting a review board.

Title: ACR X CLB THROUGH OCCUPIED ALT HAD LTSS FROM CPR G3. SYS ERROR.
Narrative: I AM BEING WRONGLY ACCUSED AS THE PRIMARY CTLR AT FAULT IN THIS SIT. DUE TO THE INADEQUACIES OF THE RADAR CTLR, WHO FAILED TO SEPARATE THESE ACFT, OUR QUALITY ASSURANCE OFFICE AT THIS TIME IS HOLDING ME RESPONSIBLE. I WILL BE REQUESTING A REVIEW BOARD.
