Narrative:

This reporter saw a news clipping in his local county newspaper. He offered it for our interests as he states it was never reported to the FAA. In preparing the aircraft for department, anti-ice fluid was being applied but the air conditioning system was not turned off. It is common for this type of incident to occur whenever the air conditioning if still running. Should any part of the anti-ice fluid enter the air distribution ducts an incident happens just as reported the newspaper graphic.
