Narrative:

Notified by flight attendant that she had not advised cockpit crew that the cabin was ready for takeoff. The cabin was not ready. Flight attendants were standing and overhead bin was open. Before takeoff checklist item is 'cabin ready' checklist was read and responded to as ready. I assumed this. There was a breakdown in cockpit resource management. Possibly poor cockpit discipline.

Narrative: NOTIFIED BY FLT ATTENDANT THAT SHE HAD NOT ADVISED COCKPIT CREW THAT THE CABIN WAS READY FOR TKOF. THE CABIN WAS NOT READY. FLT ATTENDANTS WERE STANDING AND OVERHEAD BIN WAS OPEN.
