Narrative:

Flight from paradise island to bimini resulted in termination of the flight once we had arrived in bimini. Due to lack of communication between captain and copilot only required fuel and reserve was on board for bimini. Continuation of the flight to miami was interrupted because upon arrival in miami, legal reserve would not be available. No fuel is ordinarily available in bimini. Callback conversation with reporter revealed the following information: reporter states this was a very bad day, all day. There was a lot of paper work at departure, there was a lot of extra baggage and the flight crew was helping with all of this. With the distrs both flight crew and fueling crew neglected to refuel. There was enough fuel to get to miami but the winds were high and they felt it would be a chance of coming up real short of fuel or eating into their reserve fuel. The company sent another aircraft over with fuel and the flight continued on. Incident has been reported to FAA through a company self disclosure program which gives immunity if the act was not intentional. Reporter sent in his report in addition because sometimes the self disclosure has not worked well.

Title: COMMUTER FLC FAILS TO REFUEL PRIOR TO DEPARTING FOR AN ARPT THAT DOES NOT HAVE FUEL.

Narrative: FLT FROM PARADISE ISLAND TO BIMINI RESULTED IN TERMINATION OF THE FLT ONCE WE HAD ARRIVED IN BIMINI. DUE TO LACK OF COM BTWN CAPT AND COPLT ONLY REQUIRED FUEL AND RESERVE WAS ON BOARD FOR BIMINI. CONTINUATION OF THE FLT TO MIAMI WAS INTERRUPTED BECAUSE UPON ARR IN MIAMI, LEGAL RESERVE WOULD NOT BE AVAILABLE. NO FUEL IS ORDINARILY AVAILABLE IN BIMINI. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: RPTR STATES THIS WAS A VERY BAD DAY, ALL DAY. THERE WAS A LOT OF PAPER WORK AT DEP, THERE WAS A LOT OF EXTRA BAGGAGE AND THE FLC WAS HELPING WITH ALL OF THIS. WITH THE DISTRS BOTH FLC AND FUELING CREW NEGLECTED TO REFUEL. THERE WAS ENOUGH FUEL TO GET TO MIAMI BUT THE WINDS WERE HIGH AND THEY FELT IT WOULD BE A CHANCE OF COMING UP REAL SHORT OF FUEL OR EATING INTO THEIR RESERVE FUEL. THE COMPANY SENT ANOTHER ACFT OVER WITH FUEL AND THE FLT CONTINUED ON. INCIDENT HAS BEEN RPTED TO FAA THROUGH A COMPANY SELF DISCLOSURE PROGRAM WHICH GIVES IMMUNITY IF THE ACT WAS NOT INTENTIONAL. RPTR SENT IN HIS RPT IN ADDITION BECAUSE SOMETIMES THE SELF DISCLOSURE HAS NOT WORKED WELL.
