Narrative:

Well it was bound to occur, we had just closed up the #1, had instructed me after door closure that the cabin was ready and had announced the doors to be armed. The next thing I knew was the jet bridge back up to the aircraft. My first officer called back that they were going to open the door to the extra flight attendant. She turned after hearing the agent pat on the door, she cracked the forward entry lever without checking the slide and the agent pulled the door open. Luckily the slide fell out without hanging up. The pin or lanyard came about 1/8 inch from deploying. A close call. The error was the extras rush to open the door without checking the slide. She did say she did not hear the announcement to arm the slides, then said she never armed the slides at the gate, then said the first officer said hurry up. He did say they're going to open the door and she simply rushed. Close call. I'll bet she'll never do it again. Maintenance came, restored the slide. Operations normal from there.

Title: MLG CABIN ATTENDANT OPENS DOOR WITH SLIDE ARMED.

Narrative: WELL IT WAS BOUND TO OCCUR, WE HAD JUST CLOSED UP THE #1, HAD INSTRUCTED ME AFTER DOOR CLOSURE THAT THE CABIN WAS READY AND HAD ANNOUNCED THE DOORS TO BE ARMED. THE NEXT THING I KNEW WAS THE JET BRIDGE BACK UP TO THE ACFT. MY FO CALLED BACK THAT THEY WERE GOING TO OPEN THE DOOR TO THE EXTRA FLT ATTENDANT. SHE TURNED AFTER HEARING THE AGENT PAT ON THE DOOR, SHE CRACKED THE FORWARD ENTRY LEVER WITHOUT CHKING THE SLIDE AND THE AGENT PULLED THE DOOR OPEN. LUCKILY THE SLIDE FELL OUT WITHOUT HANGING UP. THE PIN OR LANYARD CAME ABOUT 1/8 INCH FROM DEPLOYING. A CLOSE CALL. THE ERROR WAS THE EXTRAS RUSH TO OPEN THE DOOR WITHOUT CHKING THE SLIDE. SHE DID SAY SHE DID NOT HEAR THE ANNOUNCEMENT TO ARM THE SLIDES, THEN SAID SHE NEVER ARMED THE SLIDES AT THE GATE, THEN SAID THE FO SAID HURRY UP. HE DID SAY THEY'RE GOING TO OPEN THE DOOR AND SHE SIMPLY RUSHED. CLOSE CALL. I'LL BET SHE'LL NEVER DO IT AGAIN. MAINT CAME, RESTORED THE SLIDE. OPS NORMAL FROM THERE.
