Narrative:

During an EMS flight my radar altimeter was working intermittently as well as on postflt checks I found the pilot pitot heat inoperative. I informed our 2 a&P's as well as my relief pilot. However, I got caught up in non flying management tasks and neglected to enter the problems in the aircraft logbook. The pitot heat problem turned out to be intermittent as well. Applicable parts were ordered at the time the a&P's were notified. By getting side tracked soon after landing, I allowed myself to neglect an important part of each flight -- the follow-up. As they say, 'the job isn't done until the paper work is complete.'

Title: ATX ROTARY WINGED PLT FORGETS TO MAKE A LOGBOOK ENTRY ON ACFT EQUIP PROBS.

Narrative: DURING AN EMS FLT MY RADAR ALTIMETER WAS WORKING INTERMITTENTLY AS WELL AS ON POSTFLT CHKS I FOUND THE PLT PITOT HEAT INOP. I INFORMED OUR 2 A&P'S AS WELL AS MY RELIEF PLT. HOWEVER, I GOT CAUGHT UP IN NON FLYING MGMNT TASKS AND NEGLECTED TO ENTER THE PROBS IN THE ACFT LOGBOOK. THE PITOT HEAT PROB TURNED OUT TO BE INTERMITTENT AS WELL. APPLICABLE PARTS WERE ORDERED AT THE TIME THE A&P'S WERE NOTIFIED. BY GETTING SIDE TRACKED SOON AFTER LNDG, I ALLOWED MYSELF TO NEGLECT AN IMPORTANT PART OF EACH FLT -- THE FOLLOW-UP. AS THEY SAY, 'THE JOB ISN'T DONE UNTIL THE PAPER WORK IS COMPLETE.'
