Narrative:
A H/B767 was 4 mi final to runway 4R on an ILS approach behind a B757 on a 1 mi final. Loss of separation was caused by a workload too heavy to accommodate the number of aircraft and mi required for standard separation. In addition the approach controller kept the air carrier X on his frequency until the aircraft was 4 mi out, leaving no option available to the tower controller to separate the 2 aircraft. Visual approachs were available but not used by the approach controller.

Title: OP DEV OPERROR.
Narrative: A H/B767 WAS 4 MI FINAL TO RWY 4R ON AN ILS APCH BEHIND A B757 ON A 1 MI FINAL. LOSS OF SEPARATION WAS CAUSED BY A WORKLOAD TOO HVY TO ACCOMMODATE THE NUMBER OF ACFT AND MI REQUIRED FOR STANDARD SEPARATION. IN ADDITION THE APCH CTLR KEPT THE ACR X ON HIS FREQ UNTIL THE ACFT WAS 4 MI OUT, LEAVING NO OPTION AVAILABLE TO THE TWR CTLR TO SEPARATE THE 2 ACFT. VISUAL APCHS WERE AVAILABLE BUT NOT USED BY THE APCH CTLR.
