Narrative:

As we lifted off the runway we heard the aircraft taking position (behind us) report rubber and metal debris on the runway we had just departed. Left gear down and leveled off to hear what it was -- tire and landing gear door. Suspected it was ours. Told flight attendants we may have a problem and they confirmed hearing noise at start of takeoff roll. Vectored in holding pattern for about 1 hour while we conferred with maintenance, dispatch, etc, burned fuel down to landing weight and prepared the cabin for a possible evacuate/evacuation. Landed normally at mia (longer runways, better emergency facilities) after a low pass to check gear (nothing abnormal seen from tower nor from another air carrier). Taxied off and stopped with fire trucks/maintenance observing -- no fire -- so we received a tow to the gate. No injuries but substantial damage to main landing gear, flaps and underside of wing area. Company had reported defective tires on this fleet. Cockpit resource management training worked well. A company omc and captain worked with maintenance/dispatch and cabin crew while I (copilot) flew the airplane and monitored ATC. Fortunately, WX was good and traffic light. Felt we had plenty of time to prepare, do procedures and make a good decision. Captain was a good manager and teamwork was strong. Would like to emphasize for flight attendants to be more willing to report irregularities to cockpit -- although we were beginning the takeoff so there was very little time to do this before high speed regime made it inappropriate.

Title: BLOWN TIRE ON TKOF.

Narrative: AS WE LIFTED OFF THE RWY WE HEARD THE ACFT TAKING POS (BEHIND US) RPT RUBBER AND METAL DEBRIS ON THE RWY WE HAD JUST DEPARTED. WOULD LIKE TO EMPHASIZE FOR FLT ATTENDANTS TO BE MORE WILLING TO RPT IRREGULARITIES TO COCKPIT -- ALTHOUGH WE WERE BEGINNING THE TKOF SO THERE WAS VERY LITTLE TIME TO DO THIS BEFORE HIGH SPD REGIME MADE IT INAPPROPRIATE.
