Narrative:

I failed to send an amendment to the dispatch release. The amendment should have added 200 pounds to the minimum fuel required for the flight. The flight was completed without the additional 200 pounds of fuel that was required due to a cdl. The captain pointed the problem out to me and I made the required adjustment to the minimum fuel on the flight's second segment. I had adjusted the flts minimum fuel in my computer system prior to the flight's departure but simply forgot to send an amended release to the affected departure stations.

Title: DISPATCHER FAILED TO INCLUDE AN AMENDMENT ON THE RELEASE FOR 200 LBS OF FUEL.

Narrative: I FAILED TO SEND AN AMENDMENT TO THE DISPATCH RELEASE. THE AMENDMENT SHOULD HAVE ADDED 200 LBS TO THE MINIMUM FUEL REQUIRED FOR THE FLT. THE FLT WAS COMPLETED WITHOUT THE ADDITIONAL 200 LBS OF FUEL THAT WAS REQUIRED DUE TO A CDL. THE CAPT POINTED THE PROB OUT TO ME AND I MADE THE REQUIRED ADJUSTMENT TO THE MINIMUM FUEL ON THE FLT'S SECOND SEGMENT. I HAD ADJUSTED THE FLTS MINIMUM FUEL IN MY COMPUTER SYS PRIOR TO THE FLT'S DEP BUT SIMPLY FORGOT TO SEND AN AMENDED RELEASE TO THE AFFECTED DEP STATIONS.
