Narrative:

Reference pre departure clearance messages. I accepted an old pre departure clearance from the gate agent's teletype with several factors lulling me not to check the correct flight number (initially handing it to the first officer without looking at it, checking only the correct route, SID, destination, altitude, timing of message from agent). Takeoff was made with wrong transponder code. Problems could be worse if route changes are made to teletype pre departure clearance's and no readbacks of any kind are made with ATC, i.e., phl. Ord has better system. Some kind of pre departure clearance verification should be required before takeoff.

Title: WRONG PDC, AGAIN.

Narrative: REF PDC MESSAGES. I ACCEPTED AN OLD PDC FROM THE GATE AGENT'S TELETYPE WITH SEVERAL FACTORS LULLING ME NOT TO CHK THE CORRECT FLT NUMBER (INITIALLY HANDING IT TO THE FO WITHOUT LOOKING AT IT, CHKING ONLY THE CORRECT RTE, SID, DEST, ALT, TIMING OF MESSAGE FROM AGENT). TKOF WAS MADE WITH WRONG XPONDER CODE. PROBS COULD BE WORSE IF RTE CHANGES ARE MADE TO TELETYPE PDC'S AND NO READBACKS OF ANY KIND ARE MADE WITH ATC, I.E., PHL. ORD HAS BETTER SYS. SOME KIND OF PDC VERIFICATION SHOULD BE REQUIRED BEFORE TKOF.
