Narrative:

On arrival at gate parking brake was left set because ground agent left red stop light on. At pushback brake pedals were pushed but brakes did not release. As tug began to push, tow bar failed and damaged aircraft nose gear spray deflector and then slid under nose gear tires. Parking brake was still set. I have reevaluated my prepush activity to insure that I do not make this mistake again.

Title: CAPT OF AN MLG ACR ACFT FAILED TO RELEASE THE PARKING BRAKE FOR PUSHBACK RESULTING IN THE TOW BAR DAMAGING THE NOSE GEAR SPRAY DEFLECTOR AND LODGING UNDER THE NOSEWHEEL TIRES.

Narrative: ON ARR AT GATE PARKING BRAKE WAS LEFT SET BECAUSE GND AGENT LEFT RED STOP LIGHT ON. AT PUSHBACK BRAKE PEDALS WERE PUSHED BUT BRAKES DID NOT RELEASE. AS TUG BEGAN TO PUSH, TOW BAR FAILED AND DAMAGED ACFT NOSE GEAR SPRAY DEFLECTOR AND THEN SLID UNDER NOSE GEAR TIRES. PARKING BRAKE WAS STILL SET. I HAVE REEVALUATED MY PREPUSH ACTIVITY TO INSURE THAT I DO NOT MAKE THIS MISTAKE AGAIN.
