Narrative:

Jan/sat/94, approximately XJ00, md-80 jet airliner was being lowered from all 3 jacks (2 wing and 1 tail). As the aircraft was being lowered the tail jack failed and aircraft slipped off it going up through the aircraft causing major damage, mechanics were almost killed. As far as mechanics here at phl know, (air carrier) does not have any type of preventive maintenance and checks for their aircraft jacks. This we feel is very dangerous for everyone, the mechanics, the company and especially the aircraft that are on jacks. This aircraft was jacked properly by 4 seasoned aircraft mechanics by the book (maintenance. Manual) procedures. The jack failure occurred during the lowering of the aircraft off the jacks. I suggest that (air carrier) start a monthly check and preventive maintenance program on all their aircraft jacks here in philadelphia, PA. To prevent this accident from happening again. This is a very dangerous situation we have here in philadelphia, PA. Someone almost got killed.

Title: AN MD-80 ACFT FALLS ON JACK IN MAINT CAUSING FUSELAGE TO BE PUNCTURED.

Narrative: JAN/SAT/94, APPROX XJ00, MD-80 JET AIRLINER WAS BEING LOWERED FROM ALL 3 JACKS (2 WING AND 1 TAIL). AS THE ACFT WAS BEING LOWERED THE TAIL JACK FAILED AND ACFT SLIPPED OFF IT GOING UP THROUGH THE ACFT CAUSING MAJOR DAMAGE, MECHS WERE ALMOST KILLED. AS FAR AS MECHS HERE AT PHL KNOW, (ACR) DOES NOT HAVE ANY TYPE OF PREVENTIVE MAINT AND CHKS FOR THEIR ACFT JACKS. THIS WE FEEL IS VERY DANGEROUS FOR EVERYONE, THE MECHANICS, THE COMPANY AND ESPECIALLY THE ACFT THAT ARE ON JACKS. THIS ACFT WAS JACKED PROPERLY BY 4 SEASONED ACFT MECHS BY THE BOOK (MAINT.
